The Prenatal Plus program is a special program for at-risk pregnant women. If you are enrolled in the Prenatal Plus program, you will be provided with case management, nutrition counseling, and psycho-social services. The services are meant to lower risk for negative maternal and infant health outcomes due to lifestyle, behavioral, and other non-medical parts of a woman’s life that could affect her pregnancy.
- Pediatric Personal Care Services – Colorado Department of Health Care Policy and Finance State
Pediatric Personal Care services help Health First Colorado (Colorado’s Medicaid program) members with physical, maintenance and supportive needs such as bathing, dressing, meal preparation and medication reminders. There are no costs, pregnant woman, and children are not charged co-pays.

- Intermountain Nurse Family Partnership State
Nurse Family Partnership (NFP)is a free program pairing first time expecting mothers with an experienced nurse home visitor from early pregnancy until the infant is 2 years of age. The NFP program empowers and assists women in their transition to motherhood by addressing overall health of mom and baby, prenatal concerns, child development, maternal-child bonding, understanding baby cues, nutrition, mental health, postpartum depression, community resources, and maternal self-sufficiency.
- Nure Home Visitation – Jefferson County State
Collaboration nurses offer parenting support, stress management tools and information regarding nutrition, feeding and care of children, birth control and behavioral issues. Nurses can offer support to moms, dads, grandparents and other caregivers and provide services on average from 4 to 7 months.
- Denver Health Nurse Family Partnership State
The Nurse Family Partnership is a national program for low-income women who are having their first baby. A registered nurse visits the mother in her home every two weeks during pregnancy and up until the child is two years old to ensure that the mom and baby are healthy and doing well. The program is voluntary, and women can enroll themselves in the program.

They also offer an online course for our clients called Bright Course with incentives as they finish the course.

- CHP+ Prenatal Program State
It is free to enroll in the CHP+ Prenatal Care Program. All pregnancy care is free, including prenatal care, labor and delivery, and visits after you deliver. Other medical care, such as prescriptions, may have small co-payments depending on your family size and income. If you are found eligible, your coverage will last for at least 60 days after your pregnancy ends. Coverage will end 60 days after the last day of the month when your pregnancy ended. If you are pregnant, you may qualify for a program called Presumptive Eligibility (PE). PE is a way for pregnant women to receive prenatal care right away without having to wait to see if you are eligible.

Special Connections helps women have healthier pregnancies and healthier babies by providing case management, counseling, and health education during pregnancy and up to one year after delivery. Special Connections services are in addition to any prenatal care you receive from her doctor or nurse-midwife.

Presumptive Eligibility (PE) is a program that gives temporary medical coverage right away. The coverage starts the same day you apply and lasts for at least 45 days while they process your Medical Assistance application. You must be a child under 19 or a pregnant woman.

Pediatric Personal Care services help Health First Colorado (Colorado’s Medicaid program) members with physical, maintenance and supportive needs such as bathing, dressing, meal preparation and medication reminders. There are no costs, pregnant woman, and children are not charged co-pays.
- Emergency Medicaid – Colorado Department of Health Care Policy and Finance State
Emergency Medicaid provides short-term Health First Colorado (Colorado’s Medicaid program) coverage for eligible people who do not meet immigration or citizenship requirements for Medicaid and need treatment for life-threatening medical emergencies. It is sometimes also called “Emergency Medical Services,” or “EMS.”

The Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit provides comprehensive and preventive health care services. EPSDT is key to ensuring that children and youth receive appropriate preventive, dental, mental health, developmental and specialty services.
- Health First Colorado Dental Benefits – Colorado Department of Health Care Policy and Financing State
Dental services are a program benefit for enrolled Health First Colorado (Colorado’s Medicaid program) members of all ages.
- Colorado Indigent Care Program – Colorado Department of Health Care Policy and Financing State
They administer the Colorado Indigent Care Program (CICP), which provides discounted health care services to low-income people and families. CICP is not a health insurance program. Discounted health care services are provided by Colorado hospitals and clinics that participate in the CICP.
- CHP+ Prenatal Program State
It is free to enroll in the CHP+ Prenatal Care Program. All pregnancy care is free, including prenatal care, labor and delivery, and visits after you deliver. Other medical care, such as prescriptions, may have small co-payments depending on your family size and income. If you are found eligible, your coverage will last for at least 60 days after your pregnancy ends. Coverage will end 60 days after the last day of the month when your pregnancy ended. If you are pregnant, you may qualify for a program called Presumptive Eligibility (PE). PE is a way for pregnant women to receive prenatal care right away without having to wait to see if you are eligible.
- Child Health Plan Plus State
Child Health Plan Plus (CHP+) is public low-cost health insurance for certain children and pregnant women. It is for people who earn too much to qualify for Health First Colorado (Colorado’s Medicaid Program), but not enough to pay for private health insurance.
- Colorado Childcare Assistance Program State
The Colorado Child Care Assistance Program (CCCAP) helps families that are homeless, working, searching for work, or in school to find low-cost child care assistance.
- Colorado Works State
The Colorado Works Program serves low-income families with children by providing income assistance and supportive services to help participants become and remain self-sufficient. Colorado Works is the new program that replaced the Aid to Families with Dependent Children (AFDC) and JOBS programs. Benefits may be provided for single and two parent families, pregnant women, and relatives caring for minor children. Colorado Works benefits are available to parents with children who are working or involved in a work preparedness activity.
