Nurse-Family Partnership helps transform the lives of mothers and their babies. Registered nurses make regular home visits and provide moms with the care and support they need to have a healthy pregnancy, provide responsible and competent care for their children, and become more economically self-sufficient.

Family Connects is a nurse-based approach to supporting newborns and their families through nurse home visits, connecting families to services in the community, and supporting families with an individual approach to care.

Who We Serve

• Prenatal: pregnant moms and expectant families
• Ages 0-3: families with babies and toddlers

How it Works

• Home Visiting: in-home coaching with trained parent educators
• 2 Generation Approach: education and support for the whole family
